Transitions, St. Lawrence County


Assists individuals ages 15 through 30 with developmental disabilities in setting goals and identifying areas of interest. Areas include self advocacy, meal preparation, chores, money skills, grocery shopping, decision making, self direction, and interpersonal skills.

Eligibility: Individuals between the ages of 15 and 30 with an intellectual or developmental disability
